This is especially important for:
• 4-year-olds entering pre-kindergarten
• 5-year-olds entering kindergarten
• 11-year-olds entering seventh grade
• Students catching up on required vaccines
📌 If your child has a vaccine exemption, please make sure the exemption form is current. Exemption forms expire every two years.
📌 Students must be compliant with all required vaccines in order to start school.
Important reminders:
A printed current vaccine record or school note listing missing vaccines is required. Electronic proof cannot be accepted. A parent or guardian must be present.
The clinic is available for Godley ISD students ages 3-18. Older children who resist shots and children with special needs should see their pediatrician. This clinic does not replace regular well-child checkups.
Staff and supplies are limited, and the clinic may end early if supplies or time runs out.
Payment/insurance:
Accepted PPO plans include Aetna, Blue Cross Blue Shield, Cigna, Humana and United Healthcare. A paper copy of the insurance card and driver’s license must be provided. Medicaid/CHIP is accepted at no charge with a paper copy of the card.
Uninsured students: $12 per vaccine, with a $48 maximum per child. Cash or Zelle only. Bills over $20 cannot be accepted.
Vaccines available:
Chickenpox, DTaP, Hepatitis A, Hepatitis B, Hib, HPV, meningitis, meningitis B, MMR, polio, pneumonia and Tdap.
