Immunization Clinics - Back to School 2025
Put your child's health on your Back to School list!
We will need new medication forms (prescription/over the counter), asthma, seizure, diabetic and allergy action plans. A doctor’s note is required if your student requires any medical care at school. It will now be required to have a parent note and your own medication to take Ibuprofen or Tylenol. We will not have a supply at school to give your child. They will be allowed to take it 3 times with us, then will be required to get a doctor’s note to continue taking it. Click on the link below to access health forms.
Make sure your child is up to date on all vaccines that are required to start school.
- 4 year olds going to Pre-kindergarten
- 5 year olds going to Kindergarten
- 11 year olds going into 7th grade
- Students catching up on vaccines
**If your child is exempt from vaccines, please make sure their exemption form is up to date. They expire every 2 years.
***Students will need to be compliant with all required vaccines in order to start school. We will hold a vaccine clinic on July 17 and July 31-August 1, clinic flyers are posted below. Please email kjokel@godleyisd.net with any questions or concerns. Have a wonderful summer!
