Action Needed: SB12 Parental Consent for Each Student

Godley ISD asks primary guardians to complete the SB12 Parental Consent form in Skyward Family Access for each student.

Parental Consent Notice

The notice explains the health and counseling services available to support students’ learning and well-being:

  • Health services: Routine nursing care, first aid for injuries and illness, medication administration, vision and hearing screenings and other required health screenings, management of chronic conditions such as diabetes, seizures and allergies, and coordination of health services.

  • Counseling services: Academic and career planning, short-term individual counseling and check-ins, support during emotional distress or grief, help with managing emotions and building positive relationships, problem-solving and conflict resolution, and prevention and intervention services.

  • Parents’ rights: Information about consent, notification and access to student records.

The notice describes available services; it does not mean every service will be provided to your child.

Please review the attachment, then follow these steps:

  1. Go to www.godleyisd.net/skyward and log in to Skyward Family Access as the student’s primary guardian.

  2. Locate SB12 Parental Consent, the last tile on your home page.

  3. Click the tile and scroll down to review the form.

  4. Select Yes or No and sign the form.

  5. Click Submit at the top.

Repeat these steps for each student in your family.

Consent remains effective through the end of the school year unless revoked. Parents may revoke or reinstate consent at any time by providing signed written notice. Certain emergency responses and legally required activities do not allow parents to opt out, as explained in the attachment.
