- Please bring a copy of your insurance card and primary insured's driver's licence
- Aetna, BCBS, Cigna, Humana & United Healthcare PPO type plans are accepted. Do no accept HMO plans.
- Without accepted insurance plans: Adults $30/flu shot, Children under 18 $10/flu shot - Accepted payments are cash and Zelle
