A healthy student is a ready-to-learn student! As you prepare for the new school year, take a few minutes now to make sure your child's health information is up to date. Completing required forms and verifying immunization records before the first day of school helps our nurses provide the best care and ensures a smooth start for your student.

Before the first day of school, please:
✔ Complete and submit all required medical forms to your campus nurse.
✔ Update any medication authorization forms or emergency action plans, if applicable.
✔ Verify your child's immunization records are current. Texas law requires students to be up to date on all required vaccines to attend school.
