UIL Required Forms & Physicals
To participate in athletics at Godley all student-athletes must have the required UIL paperwork and a valid physical on file.
Physicals are required every 2 years, unless a significant or emergent health event occurs—in which case a new physical will be required before the athlete can return to participation. Physicals must be completed on the official UIL Pre-Participation Physical Evaluation form.
The Medical History Form (front page of the physical form) must be completed every year,
even if a new physical is not required.
In addition, student-athletes must complete the annual UIL paperwork packet, which includes:
- Steroid Agreement Form
- Sudden Cardiac Arrest Awareness Form
- Concussion Acknowledgment Form
All forms—including the medical history and UIL packet—can be completed online through Rank One.
UIL Physical Requirements
If your student plans to participate in athletics, cheer or band during the 2026-2027 school year, now is the time to check UIL physical requirements!
📌 NEW PHYSICALS ARE REQUIRED FOR:
• Incoming 7th graders
• Incoming 9th graders
• Incoming 11th graders
📌 STUDENTS IN ANY GRADE LEVEL WHO MAY ALSO NEED A PHYSICAL:
• Students who did NOT participate during the 2025-2026 school year
• Students whose current physical is expiring
• Students who have had a major surgery or medical event since their last physical
UIL requires a new physical every two years for participation in athletics, cheer and band.
