Athletic Physicals for 2026-27 School Year

Godley ISD will host a FREE Physical Clinic for student-athletes, cheerleaders, and band members on Wednesday, April 29, 2026, at the Godley High School Main Arena. 

Clinic Times

2:00 PM – 4:00 PM
Priority will be given during this window to:

• Bus riders who may not have transportation home
• Junior High athletes

4:00 PM - 6:00 PM
Open to all eligible students.

Eligible Students

The free physical clinic is available for:

• Incoming 7th graders who plan to participate in athletics, cheer, or band
• Current 7th grade through juniors participating in athletics, cheer, or band
• Students whose physical is expiring
• Students who have had a major surgery or major medical event since their last physical and are required to get a new one before the 2 year window

UIL Physical Requirements

UIL requires a new physical every two years. This means new physicals are required for:

• Incoming 7th graders
• Incoming 9th graders
• Incoming 11th graders

Students outside of those grade levels will also need a new physical if their current one is expiring. Students who have experienced a major surgery or major medical event since their last physical are also required to get a new one.

We will do our best to pull a list of student-athletes who are outside of the normal windows and send a message to parents via parent square to give you a heads up! 

Required Forms

Athletes must bring:

• A physical form
• The medical history form completed and signed by a parent/guardian (front page of physical form)

Junior High athletes who plan to be seen without a parent/guardian present must also have a signed permission slip turned in ahead of time. These can be emailed directly to the athletic trainer, turned directly into a coach or turned in at the front office. 

Permission slips will be sent home with all athletes and will also be posted on the website under Free Physical Clinic Information 

We will have blank copies at the clinic for any student-athletes coming with a parent/guardian

Important Paperwork Information

To help prevent lost paperwork:

• No physical forms will leave the clinic once completed
• Copies will be kept and uploaded into Rank One
• Band Director will receive copies of all band members for their records as well
