If you feel you already possess the skills to complete the evaluation document, you may produce the document and email it to Kelly Hanna (khanna@godleyisd.net). She will see if you have correctly produced the document and will send a signed evaluation to you. You will need to make a copy for yourself and give the original to your principal.
